\/>Co-amoxiclav has the highest reported rate of drug-induced cholestatic hepatitis of any oral antibiotic \u2014 approximately 1 in 2,500 to 1 in 6,000 courses. Onset is characteristically <strong>delayed (typically 1\u20136 weeks after the course finishes)<\/strong>, with jaundice, dark urine, pale stool, and pruritus. Prior episode of co-amoxiclav-associated jaundice is an <strong>absolute lifelong contraindication<\/strong> \u2014 recurrence risk on re-exposure is very high. Risk is greater in older men and with prolonged courses.<\/div>\n<h2>What Augmentin is for<\/h2>\n<p>Augmentin combines amoxicillin (a broad-spectrum aminopenicillin) with clavulanic acid (an irreversible beta-lactamase inhibitor). The combination retains amoxicillin&rsquo;s gram-positive and selected gram-negative coverage while adding activity against beta-lactamase-producing strains of <em>Staphylococcus aureus<\/em> (methicillin-sensitive only), <em>Haemophilus influenzae<\/em>, <em>Moraxella catarrhalis<\/em>, <em>Klebsiella<\/em>, anaerobes including <em>Bacteroides fragilis<\/em>, and many <em>Escherichia coli<\/em>. Common indications:<\/p>\n<ul>\n<li><strong>Acute bacterial sinusitis<\/strong> \u2014 preferred when symptoms are severe, persistent &gt; 10 days, or worsening after initial improvement.<\/li>\n<li><strong>Acute otitis media<\/strong> \u2014 second-line after amoxicillin alone, or first-line when high beta-lactamase prevalence is expected.<\/li>\n<li><strong>Community-acquired pneumonia (CAP)<\/strong> \u2014 typically combined with a macrolide for atypical coverage.<\/li>\n<li><strong>Acute exacerbations of COPD<\/strong> with purulent sputum and increased dyspnoea.<\/li>\n<li><strong>Complicated UTI \/ pyelonephritis<\/strong> in selected cases, guided by local resistance and culture.<\/li>\n<li><strong>Skin and soft-tissue infections<\/strong> including animal-bite and human-bite prophylaxis (covers <em>Pasteurella multocida<\/em>, <em>Eikenella corrodens<\/em>, oral anaerobes).<\/li>\n<li><strong>Selected bacterial GI infections<\/strong> in traveller&rsquo;s context \u2014 not first-line for routine traveller&rsquo;s diarrhoea (azithromycin or a fluoroquinolone are preferred), but useful for documented sensitive pathogens.<\/li>\n<li><strong>Dental abscess \/ odontogenic infection<\/strong> when penicillin-V monotherapy is inadequate.<\/li>\n<\/ul>\n<div style=\"background:#fff8e1;border-left:4px solid #f5a623;padding:14px 18px;margin:18px 0;border-radius:4px;\"><strong style=\"color:#8a6d3b;\">\ud83d\udca1 Not first-line for traveller&rsquo;s diarrhoea<\/strong><br \/>Most bacterial traveller&rsquo;s diarrhoea is caused by enterotoxigenic <em>E. coli<\/em>, Campylobacter, Shigella, or Salmonella. <strong>Single-dose azithromycin 1 g<\/strong> is preferred for South \/ Southeast Asia (high Campylobacter fluoroquinolone resistance) and a 1\u20133 day fluoroquinolone course (ciprofloxacin, levofloxacin) is preferred elsewhere. Co-amoxiclav has a role in selected sensitive infections but should not be the empirical traveller&rsquo;s-diarrhoea agent of choice.<\/div>\n<h2>How Augmentin works<\/h2>\n<p><strong>\u0391\u03bc\u03bf\u03be\u03b9\u03ba\u03b9\u03bb\u03bb\u03af\u03bd\u03b7<\/strong> binds penicillin-binding proteins (PBPs) in the bacterial cell wall, disrupting peptidoglycan cross-linking and triggering cell lysis \u2014 a bactericidal effect. <strong>Clavulanic acid<\/strong> has minimal antibacterial activity of its own; it is a &#8220;suicide substrate&#8221; that irreversibly binds bacterial beta-lactamase enzymes, preventing them from hydrolysing amoxicillin&rsquo;s beta-lactam ring.
