\u03bc\u03b1\u03c2.<\/p>\n<p><strong>Celeheal<\/strong> is an oral capsule containing <strong>celecoxib<\/strong>, the first selective COX-2 inhibitor approved by the FDA (1998) and still the most widely used coxib in the world. Celecoxib is on the WHO Essential Medicines List.<\/p>\n<p>Celecoxib selectively blocks the <strong>COX-2 enzyme<\/strong> (responsible for inflammation) while largely sparing the <strong>COX-1 enzyme<\/strong> (which maintains stomach lining, kidney perfusion, and platelet function). This mechanism gives celecoxib a significantly lower risk of peptic ulcer disease and GI bleeding than classical non-selective NSAIDs such as ibuprofen, diclofenac, and naproxen. The large-scale PRECISION trial (2016) confirmed that at moderate doses celecoxib is non-inferior to ibuprofen and naproxen in cardiovascular safety, with better GI tolerance.<\/p>\n<p>Two capsule strengths are available:<\/p>\n<ul>\n<li><strong>Celeheal 100&nbsp;mg<\/strong> &mdash; osteoarthritis maintenance, first-dose in rheumatoid arthritis<\/li>\n<li><strong>Celeheal 200&nbsp;mg<\/strong> &mdash; standard dose for osteoarthritis and acute pain; half of the rheumatoid\/ankylosing spondylitis total daily dose<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\">What Is Celeheal Used For?<\/h2>\n<ul>\n<li><strong>Osteoarthritis<\/strong> &mdash; chronic joint pain, especially in patients with prior GI problems<\/li>\n<li><strong>Rheumatoid arthritis<\/strong> &mdash; symptomatic relief of active disease<\/li>\n<li><strong>Ankylosing spondylitis<\/strong> &mdash; first-line NSAID, long-term<\/li>\n<li><strong>Acute pain<\/strong> &mdash; post-surgical, musculoskeletal injury, dental<\/li>\n<li><strong>Primary dysmenorrhoea<\/strong> &mdash; when a stomach-friendly NSAID is wanted<\/li>\n<li><strong>Juvenile idiopathic arthritis<\/strong> (2 years and over, specialist supervision)<\/li>\n<li><strong>Familial adenomatous polyposis (FAP)<\/strong> &mdash; adjunctive use to reduce polyp burden (specialist GI indication)<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\">How Does Celecoxib Work?<\/h2>\n<p>Celecoxib is a <strong>selective COX-2 inhibitor (&ldquo;coxib&rdquo;)<\/strong> &mdash; it blocks the inflammation-driving COX-2 enzyme while largely sparing COX-1 (the enzyme responsible for stomach protection and normal platelet function). This gives a significantly lower risk of gastric ulcers and bleeds than classical non-selective NSAIDs.<\/p>\n<p>The therapeutic chain of events is the same for every NSAID:<\/p>\n<ol>\n<li>Tissue injury or inflammation releases phospholipids from cell membranes<\/li>\n<li>Phospholipase A&#8322; converts them to arachidonic acid<\/li>\n<li>Cyclooxygenase (COX-1\/COX-2) converts arachidonic acid to <strong>prostaglandins<\/strong> &mdash; the molecules responsible for pain, swelling, and fever<\/li>\n<li>Celecoxib blocks the COX enzymes, so less prostaglandin is produced, so there is less pain and inflammation<\/li>\n<\/ol>\n<p>Because prostaglandins also protect the stomach lining, regulate kidney blood flow, and influence platelet function, the same mechanism that relieves pain is also responsible for the main NSAID side effects: stomach irritation, fluid retention, high blood pressure, and (in some individuals) kidney stress.<\/p>\n<h2 class=\"wp-block-heading\">Celeheal Dosing<\/h2>\n<ul>\n<li><strong>Osteoarthritis:<\/strong> 200&nbsp;mg once daily or 100&nbsp;mg twice daily with food<\/li>\n<li><strong>Rheumatoid arthritis \/ ankylosing spondylitis:<\/strong> 100&ndash;200&nbsp;mg twice daily (total 200&ndash;400&nbsp;mg\/day)<\/li>\n<li><strong>Acute pain, post-operative pain, dysmenorrhoea:<\/strong> 400&nbsp;mg first dose, then 200&nbsp;mg 12 hours later if needed. Doses for twice-daily regimens should be spaced about 12 hours apart.<\/p>\n<div style=\"background:#fff3f3;border-left:4px solid #d9534f;padding:14px 18px;margin:16px 0;border-radius:4px;\">\n<p style=\"margin:0;\"><strong>GI safety &mdash; read this before the first dose.<\/strong> Every NSAID, including Celeheal, carries a real risk of gastritis, peptic ulcer, and upper-GI bleeding. The risk is highest in patients over 65, in those with prior ulcer disease, and in anyone also taking low-dose aspirin, corticosteroids, SSRIs, or anticoagulants. Take Celeheal <em>with food<\/em>, use the <em>lowest effective dose for the shortest reasonable time<\/em>, and ask your doctor about co-prescribing a proton pump inhibitor (omeprazole, pantoprazole) if you need it for more than 2&ndash;4 weeks.<\/p>\n<\/div>\n<p><strong>Note:<\/strong> Even though celecoxib has the lowest GI risk of any NSAID, GI ulcers and bleeds still occur &mdash; especially in patients &gt; 65, those on low-dose aspirin, and those on warfarin, DOACs, or SSRIs. The &ldquo;stomach-safe&rdquo; label is relative, not absolute.<\/p>\n<h2 class=\"wp-block-heading\">Who Should Not Take Celeheal?<\/h2>\n<ul>\n<li>Known hypersensitivity to celecoxib, other sulphonamides, or any NSAID<\/li>\n<li>Active or recurrent peptic ulcer or GI bleed<\/li>\n<li>Aspirin- or NSAID-induced asthma, urticaria, or angioedema<\/li>\n<li>Severe heart failure (NYHA III&ndash;IV)<\/li>\n<li>Established ischaemic heart disease, peripheral arterial disease, or cerebrovascular disease &mdash; celecoxib is generally not a first-choice NSAID in these groups<\/li>\n<li>Recent CABG surgery<\/li>\n<li>Severe hepatic impairment (Child&ndash;Pugh C)<\/li>\n<li>Severe renal impairment (CrCl &lt; 30&nbsp;mL\/min)<\/li>\n<li>Third trimester of pregnancy<\/li>\n<li>Inflammatory bowel disease flare<\/li>\n<\/ul>\n<p><strong>Sulphonamide allergy note:<\/strong> celecoxib contains a sulphonamide group. The risk is dose- and duration-dependent and is generally highest with COX-2 selective agents and with diclofenac. Patients with established ischaemic heart disease, peripheral arterial disease, stroke, or uncontrolled hypertension should use non-selective NSAIDs (ibuprofen or naproxen) at the lowest effective dose, or use paracetamol instead where possible.<\/p>\n<h3 class=\"wp-block-heading\">Renal (kidney) safety<\/h3>\n<p>NSAIDs reduce renal prostaglandin production, which can cause salt and water retention, raise blood pressure, and &mdash; in vulnerable patients &mdash; cause acute kidney injury. High-risk groups are the elderly, patients on ACE inhibitors\/ARBs plus diuretics (the &ldquo;triple whammy&rdquo;), anyone dehydrated (vomiting, diarrhoea, heat, heavy exercise), and those with pre-existing CKD.
