What Is Bacloheal?

Bacloheal is an oral tablet containing baclofen 10 mg, manufactured by Healing Pharma. Baclofen has been in clinical use since the 1970s and remains on the WHO alapvető gyógyszereinek listájára as a first-line antispasticity agent. Bacloheal is bioequivalent to the originator brand Lioresal (Novartis).

Unlike benzodiazepines, which act broadly on GABA_A receptors throughout the brain, baclofen is selective for GABA_B receptors in the spinal cord — meaning it reduces spasticity at its source with less cognitive impairment, less abuse potential and no controlled-substance restrictions.

How Does Bacloheal (Baclofen) Work?

Baclofen is a structural analogue of the inhibitory neurotransmitter GABA (gamma-aminobutyric acid). It selectively activates GABA_B receptorokhoz on presynaptic terminals in the spinal cord, which reduces the release of excitatory neurotransmitters (glutamate, aspartate). This decreases both mono- and polysynaptic reflex transmission, lowering muscle tone without eliminating voluntary movement.

• Multiple sclerosis — reduces flexor and extensor spasms, clonus, and painful muscle stiffness
• Spinal cord injury/disease — traumatic, inflammatory, degenerative, or neoplastic
• Cerebral palsy (particularly in adolescents and adults)
• Stroke-related spasticity (off-label, adjunct)
• Trigeminal neuralgia (off-label — second-line after carbamazepine)
• Alcohol withdrawal/craving (off-label — growing evidence, approved in France)

Dosing and Administration

Take with food to reduce GI side effects. Use the lowest effective dose. In renal impairment, reduce dose — baclofen is ~70–85% renally excreted.

Mellékhatások

Common (≥10%): drowsiness, dizziness, weakness/fatigue, nausea, headache, constipation.

• Abrupt withdrawal is dangerous. Sudden cessation after prolonged use (even 2–4 weeks) can trigger hallucinations, seizures, high fever, psychosis and rhabdomyolysis — a syndrome resembling neuroleptic malignant syndrome. Taper over 1–2 weeks minimum (reduce by 5–10 mg every 3–5 days).
• Renal impairment. Baclofen is 70–85% renally excreted. In CrCl <30 mL/min, accumulation causes oversedation, respiratory depression and coma. Dose-reduce and monitor closely. Some guidelines contra-indicate oral baclofen at eGFR <15.
• Older adults. Falls risk increases due to sedation and muscle weakness. Start at 5 mg once or twice daily and titrate very slowly.
• Autonomic dysreflexia. In spinal cord injury patients above T6, baclofen dose changes can precipitate autonomic dysreflexia. Titrate under specialist supervision.
• Epilepsy. Baclofen lowers the seizure threshold at high doses. Use caution in patients with a seizure history.
• Terhesség és szoptatás. Limited data. Animal studies show skeletal abnormalities at high doses. Use only if clearly needed. Baclofen is excreted in breast milk — monitor the infant for sedation.
• Driving and machinery. Baclofen causes drowsiness and slowed reactions, especially during titration. Do not drive until you know how it affects you.
• CNS depressant interactions. Additive sedation with opioids, benzodiazepines, alcohol, antihistamines and antipsychotics. Risk of fatal respiratory depression with opioids.

Contraindications — Who Should NOT Take Bacloheal

• Known hypersensitivity to baclofen or any excipient
• Active peptic ulcer disease (baclofen stimulates gastric acid secretion)
• Porfíria
• Severe renal impairment without dose adjustment and monitoring
