Meftal-P Dosing

  • Children ≥ 6 months to 14 years: approximately 6.5 mg/kg per dose, three times daily, after food. For a 15 kg child, that is one 100 mg tablet three times daily. Maximum course: 7 days.
  • Adolescents and adults: 200–300 mg (2–3 tablets) three times daily after food, maximum 7 days for acute indications. For dysmenorrhoea, the higher-strength 250 or 500 mg Meftal tablet is usually preferred.
  • Eldre: lowest effective dose; mefenamic acid is more prone to GI and renal effects in elderly patients.

Always take with food or milk. Swallow whole with water. Meftal-P tablets are small enough that they can usually be halved for paediatric weight-based dosing if necessary. For very young children, the paediatric suspension is often more convenient.

How Does Mefenamic Acid Work?

Mefenamic acid is a non-selective cyclooxygenase inhibitor (blocking both COX-1 and COX-2), reducing prostaglandin synthesis at sites of inflammation, injury, and within the uterus. Unlike other NSAIDs, mefenamic acid also has direct antagonist activity at prostaglandin receptors — which contributes to its particular effectiveness in period pain and uterine-mediated pain.

GI safety. Meftal-P (or its NSAID component) carries real risk of gastritis, peptic ulcer, and upper-GI bleeding. Risk is highest in patients over 65, in those with prior ulcer disease, and in anyone also on low-dose aspirin, corticosteroids, SSRIs, or anticoagulants. Take with food, at the lowest effective dose for the shortest reasonable time, and ask your doctor about adding a proton pump inhibitor (omeprazole, pantoprazole) for courses over 2–4 weeks.

Who Should Not Take Meftal-P?

  • Known hypersensitivity to mefenamic acid or any NSAID
  • Active peptic ulcer or history of GI bleeding
  • Inflammatory bowel disease flare (mefenamic acid is specifically associated with diarrhoea and colitis flares)
  • Severe heart failure, established ischaemic heart disease
  • Severe hepatic or renal impairment
  • Third trimester of pregnancy
  • Aspirin- or NSAID-induced asthma, urticaria, or angioedema

Bivirkninger

  • Vanlig: diarrhoea (particularly characteristic of mefenamic acid — more than other NSAIDs), dyspepsia, nausea, abdominal pain, headache, dizziness, rash
