Sjeldent, men alvorlig: severe hypoglycaemia (especially in older adults, renal impairment); heart failure decompensation; bladder cancer (long-term high-dose pioglitazone); macular oedema; bone fracture in women; lactic acidosis (very rare; acute kidney injury or hypoxia).
