What Is GTN Spray?

GTN Spray (also called nitroglycerin spray or glyceryl trinitrate spray) is a pressurised aerosol that delivers 400 mcg of glyceryl trinitrate per metered dose directly under the tongue. Each canister typically contains 200 actuations. GTN is a fast-acting nitrate vasodilator that has been the mainstay of acute angina treatment for over 100 years, originally developed from nitroglycerin synthesised in the 19th century.

The sublingual route bypasses the liver’s first-pass metabolism, delivering the active drug into the bloodstream within seconds. Clinical effect — relief of chest pain, reduction in cardiac workload, and opening of narrowed coronary arteries — typically begins within 1–3 minutes and lasts 20–30 minutes. The spray format is preferred over traditional sublingual tablets because of faster onset, easier administration (no water required), longer shelf life once opened, and more reliable dosing in patients with dry mouth.

Uses of GTN Spray

• Acute relief of angina pectoris — the primary indication. Used at the onset of chest pain or pressure from known coronary artery disease.
• Prevention of angina — taken 5–10 minutes before activities known to trigger chest pain (exertion, cold weather, emotional stress, sexual activity).
• Coronary artery spasm (variant/Prinzmetal angina) — rapid relief during vasospastic episodes.
• Acute left ventricular failure — in emergency settings to rapidly reduce preload.
• Hypertensive emergency with chest pain — as adjunctive acute therapy.

How GTN Works

Glyceryl trinitrate is metabolised in vascular smooth muscle cells to release oxid nitric (NO), the body’s principal endothelium-derived vasodilator. Nitric oxide activates guanylate cyclase, increasing intracellular cyclic GMP, which in turn causes smooth muscle relaxation across the venous and arterial systems.

The dominant clinical effect is venodilation — widening of venous capacitance vessels reduces blood return to the heart (preload), lowering ventricular wall tension and oxygen demand. Secondary coronary artery dilation improves blood flow to ischaemic myocardium, particularly in areas of partial occlusion. At higher doses, systemic arterial dilation also reduces afterload. Together these effects relieve ischaemic chest pain within minutes.

Doza și administrare

Standard adult dose: 1–2 sprays sublingually at the onset of chest pain. The spray can be repeated every 5 minutes up to a maximum of 3 doses. If chest pain persists beyond 15 minutes despite 3 sprays, call emergency services — the patient may be having a heart attack, not simple angina.

For prevention of exertional angina, take 1–2 sprays 5–10 minutes before the anticipated trigger (exercise, climbing stairs, sexual activity). Routine scheduled dosing is not recommended because of nitrate tolerance — continuous exposure reduces efficacy within 24–48 hours.

How to Use GTN Spray

1. Sit down — GTN causes blood pressure to drop and may cause dizziness or fainting if used standing.
2. Remove the plastic cap. On first use, or if not used for 1 week, prime the pump by spraying 1–2 times into the air away from the face.
3. Open the mouth and lift the tongue. Hold the canister upright and position the nozzle as close to the tongue’s underside as possible.
4. Press down firmly on the pump to deliver one metered spray. Do not inhale the spray.
5. Close the mouth immediately and hold the spray under the tongue without swallowing for at least 30 seconds.
6. If chest pain does not improve in 5 minutes, a second spray may be given. After 3 sprays with no relief, seek emergency medical care immediately.

Side Effects of GTN Spray

• Foarte frecvente: throbbing headache (especially with first-time use); flushing; dizziness or light-headedness on standing; tachycardia (fast heart rate).
• Frecvente: hypotension; nausea; weakness.
• Mai puțin frecvente: syncope (fainting) — particularly if used standing; reflex bradycardia; visible skin flushing.
• Rare dar grave: severe hypotension with reflex tachycardia, paradoxical chest pain in rare cases; methaemoglobinaemia at very high doses.

Precautions and Warnings

• Phosphodiesterase-5 inhibitors (sildenafil, tadalafil, vardenafil): ABSOLUTELY CONTRAINDICATED. The combination can cause life-threatening hypotension. Wait at least 24 hours after a PDE5 inhibitor before taking GTN (48 hours for tadalafil).
• Severe hypotension or cardiogenic shock: contraindicated.
• Severe aortic stenosis, hypertrophic obstructive cardiomyopathy: can cause dangerous drops in cardiac output.
• Raised intracranial pressure: avoid — vasodilation may worsen brain swelling.
• Recent head trauma or intracranial haemorrhage: contraindicated.

Interacțiuni medicamentoase

The most critical interaction is with Inhibitori PDE5 (sildenafil/Viagra, tadalafil/Cialis, vardenafil/Levitra) — combined use has caused fatalities. Also use caution with: antihypertensives (additive blood-pressure lowering); tricyclic antidepressants and antipsychotics (enhanced hypotension); alcohol (additive hypotension and flushing); heparin (GTN may reduce heparin anticoagulant effect if given together by IV route); riociguat (soluble guanylate cyclase stimulator — contraindicated).

Depozitare

Store below 25 °C in an upright position, away from heat and direct sunlight. The canister is pressurised — do not puncture, incinerate or expose to temperatures above 50 °C. Keep the cap on when not in use. Do not store in a car glove compartment where temperatures can spike. Check the expiry date regularly — replace promptly, as an expired GTN spray may not work when urgently needed.

How many GTN sprays can I use in one angina attack?

Use one spray at the onset of chest pain. If pain persists, a second spray can be given after 5 minutes, and a third after a further 5 minutes. If three sprays over 15 minutes do not relieve the pain, call emergency services — this may indicate a heart attack rather than simple angina.

Why does GTN cause headache?

The same vasodilation that relieves angina also widens blood vessels in the brain, producing a throbbing headache. This is often worst with the first few doses and tends to lessen with continued use. Paracetamol can help.

Can I use GTN Spray with Viagra?

No — this combination can cause fatal collapse in blood pressure. Never take GTN within 24 hours of sildenafil (Viagra) or vardenafil (Levitra), or within 48 hours of tadalafil (Cialis). If you take a PDE5 inhibitor and develop chest pain, tell the emergency team immediately so they can choose a safe alternative.

Does GTN Spray expire?

Yes. GTN Spray typically has a shelf life of 2–3 years from manufacture. Once the canister is primed (first use), potency lasts until the printed expiry date — this is a major advantage over older sublingual tablets, which lost potency within 6 months of opening. Check the date regularly and replace promptly.

What should I do if the spray does not seem to work?

First, ensure the canister is primed (1–2 sprays into the air if not used recently). Make sure you are holding it upright and not inhaling. If you have used 3 sprays over 15 minutes and pain continues, this may be a heart attack — call emergency services. Do not delay seeking help.

Can I use GTN Spray to prevent chest pain?

Yes. Taking 1 spray 5–10 minutes before a known trigger (strenuous exercise, sexual activity, exposure to cold) can prevent an attack. Routine scheduled dosing is not recommended because of the phenomenon of nitrate tolerance — continuous nitrate exposure reduces efficacy.

Is GTN Spray addictive?

GTN itself is not addictive. However, long-term routine use can lead to “nitrate tolerance” — reduced response — and rebound angina if stopped abruptly. Intermittent use (as needed or for anticipated triggers) avoids both issues.
