What Betnovate-S Ointment is and how it works

Betnovate-S Ointment contains betamethasone valerate 0.1% + salicylic acid 3%. The corticosteroid component binds the glucocorticoid receptor in keratinocytes and inflammatory cells, suppressing cytokine transcription and reducing erythema, itch, and skin proliferation. Salicylic acid is a beta-hydroxy acid that softens and dissolves the cement between corneocytes (desmolysis), thinning hyperkeratotic plaques and dramatically improving steroid penetration into thickened psoriatic skin. The combination is markedly more effective than steroid alone for chronic plaque psoriasis and hyperkeratotic eczema. Manufacturer: GSK (WHO-GMP certified).

Indications

• Chronic plaque psoriasis — limited body-surface area; the keratolytic component is critical for thick scaling plaques.
• Hyperkeratotic eczema — chronic, lichenified, scaly skin (palms, soles, scalp).
• Lichen simplex chronicus — thickened skin from chronic scratching.
• Pityriasis rubra pilaris — adjunct.

• Wash and dry the affected area. Apply emollient first if skin is very dry; wait 10 minutes.
• Apply a thin film of Betnovate-S Ointment twice daily — morning and evening — to plaques only.
• Avoid the face, eyelids, axillae, genitals, and skin folds (steroid + salicylic acid + occlusion = high atrophy and absorption risk).
• Maximum continuous use: 4 weeks on a single area without medical review.
• Maximum total amount: do not exceed 60 g per week in adults.
• Avoid occlusive dressings unless specifically directed.

Side effects

Can I use Betnovate-S Ointment on my scalp?

Yes — scalp psoriasis is a major indication. Apply at night, optionally with a shower cap (no plastic occlusion) for 30–60 minutes, then shampoo out. Once-daily or alternate-day frequency for scalp.

Why should I avoid the face with this product?

Both components are problematic on facial skin: the potent steroid causes atrophy, perioral dermatitis, and steroid rosacea; salicylic acid is highly irritating to thin facial skin. Use a non-keratolytic, milder steroid for facial inflammation.

Can children use Betnovate-S Ointment?

Generally not under 12 — children have higher relative body-surface area and more permeable skin, increasing salicylism risk. Use lower-potency steroid alone in children where indicated.

What about pregnancy?

Avoid extensive use. Limited focal application under specialist guidance is generally acceptable. Discuss with prescriber if pregnant or planning pregnancy.

Can I apply moisturiser too?

Yes — apply emollient first, wait 10 minutes, then Betnovate-S Ointment. Moisturiser before improves comfort; moisturiser immediately after can amplify absorption.

How do I taper off?

After plaque clearance, step down to once-daily, then alternate days, then a steroid-free maintenance regimen (calcipotriol-only, vitamin D analogue, or phototherapy). Abrupt cessation often leads to rebound flare.

Is Betnovate-S Ointment safe to use long-term?

No — both components have cumulative toxicity. Maximum continuous use is 4 weeks on any one area; long-term plaque psoriasis is better managed with intermittent steroid + vitamin D analogue rotation, biologics, or phototherapy.

What strength does Betnovate-S Ointment come in?
