Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women of reproductive age, characterised by oligo-/anovulation, hyperandrogenism (acne, hirsutism), and polycystic ovarian morphology on ultrasound. Modern management targets the dominant clinical concern — cycle regulation, fertility, hirsutism / acne, insulin resistance / metabolic risk — with different agents for different goals.

Important. PCOS is a clinical diagnosis (Rotterdam criteria — 2 of 3: oligo-/anovulation, hyperandrogenism, polycystic ovaries) — exclude pregnancy, thyroid dysfunction, hyperprolactinaemia, congenital adrenal hyperplasia, Cushing’s syndrome before diagnosis. Long-term cardiovascular and metabolic risk (type 2 diabetes, hypertension, sleep apnoea, NAFLD, endometrial cancer) is increased — annual review of weight, BP, lipids, HbA1c recommended. Weight loss of 5–10% (where applicable) restores ovulation in many women. Endometrial protection (regular withdrawal bleeds at least every 3 months) matters in women with prolonged amenorrhoea.
