What is Alphagan P Drop?

Alphagan P Drop is manufactured by Allergan and contains brimonidine tartrate 0.15% as a sterile ophthalmic solution. Brimonidine is a highly selective alpha-2 adrenergic agonist used to lower IOP in open-angle glaucoma and ocular hypertension. It has a unique dual mechanism — reducing aqueous humour production AND increasing uveoscleral outflow — and is one of the few glaucoma drops with a possible neuroprotective effect on retinal ganglion cells suggested by laboratory and small-cohort clinical evidence.

How It Works

Brimonidine activates alpha-2 receptors on the ciliary body, decreasing cAMP and reducing aqueous production. It also increases uveoscleral outflow. Average IOP reduction is 18–22%. The 0.15% formulation (Alphagan P) uses a Purite preservative system that breaks down in the tear film into water, oxygen, and chloride ions, making it gentler than benzalkonium chloride for long-term use.

Side Effects

Local (10–20%): conjunctival hyperaemia, allergic conjunctivitis (typically appears at 6–18 months — characteristic late onset), follicular conjunctivitis, eye pruritus, foreign body sensation, blurred vision.

Systemic: dry mouth, fatigue, drowsiness, headache, hypotension. Children (especially under 6) can develop somnolence and bradycardia.

Is Alphagan P Drop neuroprotective?

Some preclinical and small clinical studies suggest brimonidine may protect retinal ganglion cells beyond the IOP-lowering effect.
