What Is Dutas-T?
Dutas-T is a fixed-dose combination capsule containing tamsulosin hydrochloride 0.4 mg + dutasteride 0.5 mg. The combination attacks BPH via two complementary mechanisms:
- Tamsulosin (alpha-1A blocker): relaxes prostate and bladder-neck smooth muscle — rapid symptom relief within 1–2 weeks
- Dutasteride (dual 5-ARI): blocks both type-1 and type-2 5-alpha-reductase enzymes, reducing DHT by ~90% and shrinking the prostate over 3–6 months
The CombAT (Combination of Avodart and Tamsulosin) trial showed this combination reduced BPH progression, acute urinary retention, and surgery need more than either drug alone. Available strength: 0.4 mg tamsulosin + 0.5 mg dutasteride. Manufactured by Dr. Reddy's Laboratories.

Käyttöohje
- Swallow the capsule whole with water — do not crush, chew, or open. The inner contents leak onto skin and this risks exposure to women of reproductive age (dutasteride is teratogenic via skin).
- Take once daily, roughly 30 minutes after the same meal each day (usually breakfast).
- Tamsulosin effect begins within days; the dutasteride effect builds over 3–6 months. Peak combination benefit at 6–12 months.
- Do not skip doses — dutasteride has a 5-week half-life but tamsulosin effect is daily.
- Tell any ophthalmologist before cataract surgery (tamsulosin IFIS risk).
- Tell any clinician ordering PSA — dutasteride halves PSA readings.
Haittavaikutukset
Tamsulosin-related: dizziness, headache, abnormal ejaculation, rhinitis, asthenia.
Dutasteride-related: decreased libido (~4%), erectile dysfunction (~4%), reduced ejaculate volume, gynecomastia, mood changes.
Vakavat (harvinaiset): orthostatic hypotension, Intraoperative Floppy Iris Syndrome, priapism, persistent sexual side effects after discontinuation (PFS-equivalent).
Who Should Not Take Dutas-T
- Women of reproductive age — dutasteride is teratogenic. Do not handle broken capsules.
- Men with severe hepatic impairment (both components hepatically cleared)
- Orthostatic hypotension history
- Planned cataract / glaucoma surgery without telling ophthalmologist
- Strong CYP3A4 inhibitor co-therapy (ketoconazole, ritonavir)
- Known hypersensitivity to tamsulosin, dutasteride, finasteride, or sulphonamides
Lääkeaineenvaihdunta
- PDE5-estäjät: tamsulosin + sildenafil/vardenafil = additive hypotension. Tadalafil 5 mg daily is often the pragmatic choice.
- Voimakkaat CYP3A4-estäjät: raise both tamsulosin and dutasteride levels.
- Muut alfalukkersit: avoid — additive effect.
- PSA testing: dutasteride halves PSA. Double the measured value for interpretation.
Combination vs Monotherapy
The CombAT trial (4-year outcomes) showed combination therapy reduced BPH clinical progression by ~66% vs dutasteride alone and ~41% vs tamsulosin alone. Combination is preferred when prostate is enlarged (>30–40 mL) or PSA is elevated, suggesting ongoing glandular growth. For smaller prostates with pure symptom dominance, tamsulosin monotherapy is often sufficient.

Why the combination pill?
Tamsulosin and dutasteride attack BPH by different mechanisms — smooth-muscle relaxation and DHT suppression. Combination therapy reduces BPH progression more than either drug alone (CombAT trial).
How soon will symptoms improve?
Tamsulosin gives urinary symptom relief within 1–2 weeks. Dutasteride takes 3–6 months for prostate shrinkage. Full combination benefit is seen at 6–12 months.
Is there a size below which combination is unnecessary?
For prostates under 30 mL without risk factors, tamsulosin monotherapy often suffices. Combination is strongest for prostates 30 mL and above or men with elevated PSA, bothersome symptoms, and BPH-progression risk factors.
What about sexual side effects?
The dutasteride component contributes decreased libido (~4%) and erectile dysfunction (~4%) risk. The tamsulosin component contributes retrograde ejaculation. Discuss risk-benefit with a clinician, particularly if sexual function is a priority.
Can I stop taking Dutas-T abruptly?
No — BPH is progressive and symptoms return over weeks to months. Dutasteride has a 5-week half-life, so its effect clears slowly; tamsulosin effect ends within days. For safe discontinuation, discuss with a clinician.
Can I combine Dutas-T with a PDE5 inhibitor?
With care. Tamsulosin + sildenafil/vardenafil = additive hypotension risk. Tadalafil 5 mg daily is often preferred — it also treats BPH symptoms on its own.
What about PSA monitoring?
Dutasteride halves PSA readings. Always tell your clinician you are on Dutas-T before any PSA test. Convention is to double the measured PSA for interpretation.
Will Dutas-T prevent prostate cancer?
Not directly. The REDUCE trial suggested dutasteride reduced low-grade prostate cancer risk but slightly raised detection of high-grade tumours. Use dutasteride for BPH indications — not as prostate cancer prophylaxis.
