What Is Susten Vaginal Gel?
Susten Vaginal Gel is a natural progesterone vaginal gel at 8% concentration, delivered by single-use applicator sachet. The vaginal route achieves high local uterine concentration by preferential uterine uptake (the “first uterine pass” effect) while delivering less drug systemically — reducing drowsiness and GI side effects vs oral progesterone. Manufactured by Sun Pharma.

- Luteal phase support in IVF cycles: 90 mg gel daily starting at embryo transfer, continued through week 10–12 of confirmed pregnancy.
- Endometrial preparation for frozen embryo transfer (FET): programmed cycles using estradiol + vaginal progesterone.
- Threatened miscarriage in women with prior recurrent loss (PRISM trial subgroup with active bleeding and ≥3 prior miscarriages).
- Secondary amenorrhoea — off-label.
- Progesterone-deficient luteal phase in natural conception attempts — selected cases.
Käyttöohje
- Wash hands. Twist off the cap of the single-use applicator. Lie on your back.
- Insert the applicator into the vagina and squeeze the body to expel the gel. Discard the applicator.
- IVF/FET luteal support: 90 mg daily (one applicator) starting at embryo transfer; continue through week 10–12 of confirmed pregnancy.
- Best timing: same time daily, at bedtime is common — reduces leakage from upright activity.
- Some leakage is normal; use a panty liner. Do not douche.
- Avoid intercourse immediately after application; wait 6+ hours for best retention.
Vaginal vs Oral vs IM Progesterone
- Vaginal (this product): high uterine delivery, less drowsiness, convenient self-administration, some leakage.
- Oral micronized: convenient, sleep benefit, but low bioavailability and more GI effects.
- IM (susten-injection): highest and most reliable uterine levels, but painful injection site lumps.
Most IVF clinics use vaginal or IM progesterone for luteal support. Choice depends on convenience, cost, patient preference, and clinic protocol.
Haittavaikutukset
