Acute cystitis (uncomplicated lower urinary tract infection) presents with dysuria, urinary frequency, urgency, and suprapubic discomfort — most commonly caused by Escherichia coli ascending from the periurethral flora. First-line modern treatment is short-course antibiotics (3-day or single-dose regimens) combined with adequate fluid intake.

Hoe te gebruiken. Uncomplicated UTI in non-pregnant women — nitrofurantoin 100 mg BID × 3 days first-line, or fosfomycin 3 g single dose. Pregnancy or fluoroquinolone-resistant infection — fosfomycin or nitrofurantoin (avoid nitrofurantoin near term). Recurrent UTIs (≥ 3/year) — consider post-coital prophylaxis, low-dose nightly nitrofurantoin, or behavioural measures (post-coital voiding, hydration, cranberry adjunct).
Belangrijk. Symptoms suggesting upper UTI / pyelonephritis (loin pain, fever, rigors, vomiting) warrant urgent medical assessment and a longer-course antibiotic regimen. Recurrent UTIs in men are unusual and warrant urology evaluation. Persistent symptoms despite appropriate treatment, haematuria persisting after treatment, or symptoms in a catheterised patient need follow-up urine culture and possibly imaging.
