Pediculosis (head lice, body lice, pubic lice) and scabies are common parasitic infestations of skin and hair — head lice especially in school-age children, scabies in close-contact households and care homes. Permethrin — first-line for both head lice and scabies. Permethrin 5% cream for scabies (apply head-to-toe overnight, repeat in 7 days) and 1% cream rinse for head lice (apply to dry hair, leave 10 minutes, rinse, repeat in 7 days) is the most-used topical antiparasitic worldwide — high cure rates and excellent safety profile including in young children and pregnancy. Ivermectin — oral for scabies (and useful for head lice). Oral ivermectin 200 µg/kg single dose, repeated in 1–2 weeks, is first-line for crusted (Norwegian) scabies and a useful alternative to topical permethrin for typical scabies — especially useful in households / institutions where mass treatment is needed and topical compliance is difficult. Stocked as Iverjohn (3 / 6 / 12 mg) and Verpin (6 mg). Topical ivermectin lotion / cream / shampoo as Ivrea Cream, Ivrea Shampoo. Take with food; safe in adults, children > 15 kg, and most pregnancy / breastfeeding contexts (specialist advice for early pregnancy).

Lindane (gamma benzene hexachloride) — restricted second-line scabicide. Three lindane brands are stocked: Ascabiol (lotion), Gamascab Lotion, en Scabiheal Lotion (with cetrimide). Lindane has been withdrawn or restricted in the US (2009), EU (2008), and Canada (2004) over neurotoxicity (seizures, especially with overuse, broken skin, or paediatric overuse) and environmental persistence. Apply to whole body neck-down, leave 6–8 hours (NOT overnight), wash off thoroughly. Use only when permethrin and ivermectin are unavailable or have failed. Avoid in children under 6 years and during pregnancy / breastfeeding.

Hoe te gebruiken. Scabies — apply permethrin 5% cream to whole body from neck down (include between fingers, under nails, genitals, behind ears in young children), leave overnight, wash off in morning. Repeat in 7 days. Treat all household members and close contacts simultaneously even if asymptomatic. Wash bedding and clothing at 60°C or seal in plastic for 72 hours. Itching may persist for 2–4 weeks after successful treatment (post-scabietic itch — not treatment failure). Head lice — wet-comb after treatment to confirm clearance. Repeat treatment at 7 days to catch newly-hatched eggs.

Belangrijk. Crusted (Norwegian) scabies in immunocompromised or institutionalised patients needs combination therapy (permethrin + oral ivermectin, sometimes in 3+ doses) and contact tracing — outbreak control measures matter. Persistent itch despite proper treatment may be irritant dermatitis (over-application of scabicides) or true treatment failure (wrong diagnosis, missed contacts, inadequate application) — needs reassessment. Pubic lice (“crabs”) — same permethrin treatment + sexual partner notification + STI screening.
