Vulvovaginal candidiasis (vaginal thrush, yeast infection) affects up to 75% of women at some point — caused by Candida overgrowth (most often C. albicans). Symptoms include vulval itching, burning, soreness, thick white “cottage-cheese” discharge, and dyspareunia. Treatment options are oral single-dose fluconazole or topical / vaginal azole — both highly effective for uncomplicated infection.

Hoe te gebruiken. Uncomplicated vulvovaginal candidiasis (sporadic, mild-moderate, healthy non-pregnant woman) → fluconazole 150 mg single dose, OR clotrimazole 500 mg vaginal pessary single dose / 200 mg × 3 nights, OR clotrimazole vaginal cream × 7 days. Recurrent VVC (≥ 4 episodes/year) → fluconazole 150 mg every 72 hours × 3 doses (induction), then weekly 150 mg × 6 months (maintenance) — substantially reduces recurrence. Pregnancy → topical azole only (fluconazole avoided in pregnancy because of teratogenicity at high doses).

Belangrijk. Recurrent or severe vulvovaginal candidiasis warrants HbA1c testing for undiagnosed diabetes (recurrent thrush is a common type 2 diabetes presentation), HIV status review in selected cases, and culture / speciation if Candida glabrata or krusei suspected (less responsive to fluconazole). Persistent or unresponsive symptoms despite proper treatment warrant pelvic examination — bacterial vaginosis (treated differently), trichomoniasis, lichen sclerosus, contact dermatitis, and atrophic vaginitis can all mimic candidiasis. Up to 40% of self-diagnosed “thrush” is actually something else.
