Inflammatory bowel disease — sulfasalazine and 5-ASA agents. Sulfasalazine (5-ASA + sulfapyridine carrier) remains a cornerstone for mild-to-moderate ulcerative colitis and rheumatoid arthritis as a DMARD. We stock Saaz (500 mg). Mesalamine suppositories for distal proctitis are available as Pentasa Suppositories; oral mesalamine generics (Mesacol, Asacol) are listed in the IBS & IBD Treatment category. Topical / oral budesonide for ileal Crohn’s and right-sided UC (low systemic absorption, fewer steroid side-effects than prednisolone) is stocked as Budez CR.
Antibiotic / antiprotozoal agents for GI infection. Metronidazole (anti-anaerobic and antiprotozoal — Clostridium difficile, giardiasis, amoebic dysentery, bacterial vaginosis) is stocked as Flagyl Suspension for paediatric and weight-based adult dosing. Rifaximin (luminal-acting antibiotic for hepatic encephalopathy, IBS-D, traveller’s diarrhoea) is listed as Rifagut. Antiemetics and prokinetics. Metoclopramide (D2 antagonist with prokinetic effect, useful for diabetic gastroparesis, post-operative ileus, chemotherapy-induced nausea) is stocked as Reglan. Prochlorperazine (phenothiazine antiemetic for vertigo, migraine-associated nausea, hyperemesis gravidarum) is available as Stemetil. Domperidone (peripheral D2 antagonist with prokinetic effect; carries QT-prolongation signal — limit to 10 mg TID for ≤ 7 days, avoid in age >60 or cardiac disease) is stocked as Dompewal. Levosulpiride / domperidone alternative GR8-OD for functional dyspepsia.
Hepatoprotective agents. Ursodeoxycholic acid (UDCA) is the standard for primary biliary cholangitis, gallstone dissolution in selected non-calcified stones, and cholestasis of pregnancy. We stock Udimax 300 (300 mg). Silymarin / liver-tonic combinations are available as Liv 52 DS (the classic Himalaya formulation widely used for fatty liver, alcoholic hepatitis, drug-induced LFT elevation), and the IV version Maxiliv Injection for hospital use.
Digestive enzymes, probiotics, and adjuncts. Probiotic combinations help reduce antibiotic-associated diarrhoea and may shorten infectious diarrhoea episodes. We stock Probiotics 25 Billion CFU (multi-strain formula) and Vizylac Rich Sachet. Carminative / antispasmodic combinations for functional bloating are available as Gasex. Topical antifungal cream Crema Gel is occasionally used for perianal candidiasis adjacent to GI presentations.
Hoe te kiezen. Acid-related disease (heartburn, peptic ulcer, GERD) → PPIs first-line, see Behandeling van zuurbranden. Inflammatory bowel disease → sulfasalazine or 5-ASA mesalamine for mild UC; budesonide for ileal Crohn’s; biologics (not stocked here) for moderate-to-severe disease under specialist supervision. Functional dyspepsia / motility issues → prokinetic + acid suppression. Suspected GI infection → metronidazole for anaerobes / amoebae / giardia; rifaximin for SIBO and IBS-D. Liver disease → UDCA for cholestasis, hepatic-tonic adjuncts as supportive. Acute or persistent GI bleeding, severe pain, jaundice, or unexplained weight loss is a same-day specialist referral — do not self-medicate.
Belangrijk. All metoclopramide and domperidone use should be time-limited (5 days metoclopramide, 7 days domperidone in adults) per EMA / FDA restrictions on tardive dyskinesia and QT-prolongation respectively. Prolonged PPI use (>1 year) carries documented signals for B12 deficiency, hypomagnesaemia, fracture, CDI, and AKI/CKD — review necessity at 6 months. Antibiotics for C. difficile (vancomycin oral, fidaxomicin) are not stocked here — refer to specialist guidelines.
