Alpha-blockers for BPH and bladder outflow obstruction. Tamsulosin, alfuzosin, silodosin, and combination forms relieve bladder-outlet obstructive symptoms by relaxing prostate, bladder-neck, and urethral smooth muscle. Antimuscarinics for overactive bladder. Antimuscarinic agents block detrusor M3 receptor activity, reducing urgency, frequency, and urge incontinence in overactive bladder syndrome. Oxybutynin (the original M3 antagonist) is stocked as Oxyspas. Tolterodine (longer-acting alternative with less dry mouth) as Roliten en Terol La. Flavoxate (older alternative) as Urispas. Solifenacin / trospium / fesoterodine modern alternatives may also be available. Anticholinergic side-effects (dry mouth, constipation, blurred vision, cognitive impairment in elderly) are dose-related — start low, titrate up.
Beta-3 agonist for OAB. Mirabegron (a selective beta-3 adrenergic agonist that relaxes detrusor smooth muscle directly — useful when antimuscarinics fail or cause unacceptable anticholinergic side-effects, especially in elderly) is stocked as Mirago. Cleaner cognitive safety profile than antimuscarinics; small BP-rise signal warrants monitoring.
Urinary tract antibiotics — listed here for convenience. Nitrofurantoin (the recommended first-line agent for uncomplicated lower UTI) as Urivron, Uriheal, en Urifon. Single-dose fosfomycin 3 g sachet (alternative for uncomplicated cystitis, useful in pregnancy and resistance settings) as Fosirol Powder 3 G. Urolithiasis and chronic kidney disease adjuncts. Cystone (Himalaya herbal preparation traditionally used for urinary stones and crystalluria) as Cystone. Ketosteril (essential amino acid + keto-analogue tablets used in CKD-stage-3+ to reduce nitrogen load while maintaining nutrition) when in stock.
Belangrijk. New-onset haematuria, recurrent UTIs in men or older women, persistent retention, weight loss with urinary symptoms, or rising PSA needs urology evaluation. Antimuscarinics are contraindicated in narrow-angle glaucoma, severe BPH with retention, and significant cognitive impairment. Combination of alpha-blocker + PDE-5 inhibitor at full doses risks symptomatic hypotension — review dosing.
