What Selsun Suspension Treats

• Dandruff (pityriasis capitis) — non-inflammatory white-grey scalp flaking driven by an inflammatory response to Malassezia yeast.

How Selsun Suspension Works

Selenium sulphide acts on the scalp through three concurrent mechanisms:

1. Direct anti-yeast activity. Reduces colonisation by Malassezia furfur / globosa and impairs yeast cell wall function.
2. Slows corneocyte turnover. Cytostatic effect on the epidermis — the hyperproliferating scalp produces fewer flakes per unit time.
3. Sebostatic action. Reduces sebum production at the follicle, which removes part of the lipid substrate that Malassezia depends on.

How to Use Selsun Suspension

Shake the bottle well before each use — selenium sulphide is a suspension and the active settles. Wet the scalp thoroughly, dispense a teaspoon-sized amount onto the palm, and massage into the scalp for 30 seconds to build a lather. Leave the lather in contact for the full 2–5 minute window — short contact is the most common reason for failure. Rinse with running water until clear. Wash hands afterwards.

Side Effects of Selsun Suspension

Common (1–10%): mild scalp irritation, dryness, and transient hair texture change (rougher feel after rinsing — a conditioner offsets this); slight medicinal odour during application.

Uncommon (<1%): hair discoloration in dyed, grey, or blonde hair (yellow-green tint, generally transient and washes out within a few normal shampoos); excess oiliness or dryness; folliculitis; allergic contact dermatitis (red, itchy weeping scalp — discontinue).

Zeldzaam: nail discoloration if used on the body and the suspension is left under fingernails; hair shaft damage with long-overcontact times.

Systemic side effects from topical use are vanishingly rare. Selenium absorption through intact scalp skin during a 2–5 minute exposure is negligible.

Comparison With Ketoconazole Shampoo

Both work. Pick selenium sulphide if you also have or worry about pityriasis versicolor on the body, or want a different mechanism for refractory dandruff. Pick ketoconazole 2% shampoo if you have significant scalp inflammation and itch (better suited to that profile) or if you have light dyed hair. Many users with severe seborrhoeic dermatitis alternate the two between wash days.

Yes, occasionally — particularly on grey, blonde, or freshly dyed hair, where it can produce a transient yellow-green tint. Normal shampoos wash this out within a few washes. If you have a recent expensive colour, wait 7 days before the first medicated wash and consider ketoconazole 2% shampoo as a colour-friendlier alternative.

How fast does Selsun Suspension work?

Many patients see less itch after the first wash. Visible reduction in flaking takes 1–2 weeks. For pityriasis versicolor on the body, repigmentation of the affected patches can take several months — the active infection is gone within 1–2 weeks but the colour change persists until normal melanin production resumes.

Why does it smell?

Selenium compounds have a faint sulphur odour. The smell is from the active itself, not a perfume. It is most noticeable during application and largely gone after rinsing. Hair styling products mask any residual smell once dry.

Can I use Selsun Suspension for tinea versicolor on the body?

Yes — this is one of its best indications. Apply to wet skin (chest, back, shoulders), lather, leave for 5–10 minutes, then rinse. Daily for 7 days, then repeat at 1 month if needed. For widespread or stubborn cases, oral antifungal therapy (fluconazole single-dose or itraconazole short course) may be needed.

Can I leave Selsun Suspension on overnight?

Generally no — overnight contact substantially increases the risk of scalp irritation, hair shaft damage, and discoloration. The exception is the single-application overnight protocol some dermatologists use for stubborn pityriasis versicolor on the body. For the scalp, never leave on longer than 5 minutes.

Why does it tarnish my jewellery?

Sulphide compounds react with silver, gold-plate, and some other metals to form sulphide tarnish. Remove all jewellery (rings, earrings, chains) before washing. Tarnish on real silver can be reversed with a polishing cloth.

Is Selsun Suspension safe in pregnancy?

Compatible. Topical absorption from a rinse-off scalp product is negligible and the suspension has been widely used in pregnancy without reported harm. Avoid applying to broken skin and follow normal directions.

Can I combine Selsun Suspension with ketoconazole shampoo?

Yes — alternating the two between wash days is a standard regimen for severe or refractory dandruff and seborrhoeic dermatitis. They have different mechanisms so the combination is additive, not redundant. Do not mix them in the same wash.

What if my dandruff is not improving?

Check the basics — full 2–5 minute contact, 2–3× weekly induction. After 4–6 weeks of correct use without improvement, consider scalp psoriasis, contact dermatitis, or tinea capitis as differentials and consult a dermatologist. Oral antifungal therapy (Nizral or itraconazole) is sometimes needed for refractory disease.

Is selenium sulphide poisonous if accidentally swallowed?

Small accidental amounts during a wash are unlikely to cause serious harm but can cause GI upset (nausea, vomiting). Significant ingestion needs urgent medical attention — call your local poison centre or attend A&E. Keep the bottle out of reach of children.
