What is Alphagan P Drop?

Alphagan P Drop is manufactured by Allergan and contains brimonidine tartrate 0.15% as a sterile ophthalmic solution. Brimonidine is a highly selective alpha-2 adrenergic agonist used to lower IOP in open-angle glaucoma and ocular hypertension. It has a unique dual mechanism — reducing aqueous humour production AND increasing uveoscleral outflow — and is one of the few glaucoma drops with a possible neuroprotective effect on retinal ganglion cells suggested by laboratory and small-cohort clinical evidence.

Brimonidine activates alpha-2 receptors on the ciliary body, decreasing cAMP and reducing aqueous production. It also increases uveoscleral outflow. Average IOP reduction is 18–22%. The 0.15% formulation (Alphagan P) uses a Purite preservative system that breaks down in the tear film into water, oxygen, and chloride ions, making it gentler than benzalkonium chloride for long-term use.

Bijwerkingen

Local (10–20%): conjunctival hyperaemia, allergic conjunctivitis (typically appears at 6–18 months — characteristic late onset), follicular conjunctivitis, eye pruritus, foreign body sensation, blurred vision.

Systemisch: dry mouth, fatigue, drowsiness, headache, hypotension. Children (especially under 6) can develop somnolence and bradycardia.

Why does Alphagan P Drop cause an allergic reaction late?

Brimonidine is one of the few glaucoma drops with classic late-onset allergic conjunctivitis — typically appearing 6–18 months into therapy. If your eyes start to feel itchy and red months after a smooth start, this is the likely cause. Stop the drop and discuss alternatives.

Is Alphagan P Drop neuroprotective?

Some preclinical and small clinical studies suggest brimonidine may protect retinal ganglion cells beyond the IOP-lowering effect. The Low-Pressure Glaucoma Treatment Study (LoGTS) showed less visual-field progression on brimonidine vs timolol despite equivalent IOP control. The evidence is suggestive rather than definitive.

Why three times a day?

Brimonidine has an 8-hour duration of action. Three-times-daily dosing maintains 24-hour IOP control as monotherapy. As an add-on it can usually be twice daily.

Can I use Alphagan P Drop with my child?

If your child is under 2 years old, do not use Alphagan P Drop — it can cause life-threatening apnoea. For children 2–6 years, use only under specialist supervision. Children over 6 are generally treated like adults but with closer monitoring for somnolence.

What is Purite preservative?

An oxidising preservative that breaks down on contact with the tear film into water, oxygen, and chloride ions. Gentler than benzalkonium chloride for long-term daily use.

Why does Alphagan P Drop make me feel tired?

Brimonidine crosses the blood-brain barrier and activates central alpha-2 receptors (similar to clonidine), causing sedation, dry mouth, and mild hypotension. Use punctal occlusion to reduce systemic absorption.

Can I combine Alphagan P Drop with other glaucoma drops?

Yes — additive with prostaglandins, beta-blockers, and CAIs. Wait 5 minutes between bottles. Fixed combinations such as brimonidine+timolol exist.

What if I take MAO inhibitors?

Avoid the combination — additive sedation and hypotension are possible. Discuss with your prescriber.

Can I drive after using Alphagan P Drop?

Brimonidine can cause drowsiness, especially when starting or in heat. Wait until you know how it affects you before driving long distances.
