Mechanism

Polyethylene glycol 3350 is an inert hydrophilic polymer too large to be absorbed across the intestinal wall. In the colon, each PEG molecule hydrogen-bonds with several water molecules and holds them osmotically. The sodium, potassium, chloride, and bicarbonate ions in each sachet are calibrated to be isotonic with the colon lumen, so there is no net flux of water or salt across the gut wall — only the PEG-bound water is retained. Stool volume and softness increase, transit time falls, and defecation becomes easier. Onset 1–3 days; full effect at day 4–7.

Indikace

• Chronic constipation (idiopathic, opioid-induced, neurogenic)
• Faecal impaction (8 sachets/day for up to 3 days, then taper)
• Bowel preparation before colonoscopy (special low-volume regimens)
• Constipation in children > 2 years (paediatric strength)
• Chronic constipation in pregnancy and breastfeeding (no systemic absorption, well-tolerated)

Dávka

Adults: 1–3 sachets daily, each dissolved in 125 mL water. Faecal impaction: 8 sachets in 1 litre water over 6 hours, daily for up to 3 days. Children > 12 years: same as adult. Children 2–11: paediatric formulation, dose by age. Maintenance: titrate to one soft stool daily.

Vedlejší účinky

• Common: bloating, abdominal cramps, flatulence (resolve with continued use)
• Loose stool / diarrhoea (dose-related — reduce to 1–2 sachets/day)
• Nausea (rare; mostly with rapid intake of multiple sachets for impaction)
• Allergic reactions (rare): urticaria, angio-oedema
• Electrolyte disturbance with very-high-dose long-term use (rare)

Interakce léků

PEG 3350 has no significant systemic absorption and minimal pharmacokinetic interactions. However, by accelerating intestinal transit it can theoretically reduce absorption of drugs taken at the same time. Take other oral medications at least 1 hour before or 6 hours after the PEG dose, particularly thyroid hormone (levothyroxine), antiepileptics, and immunosuppressants (tacrolimus, ciclosporin) where bioavailability matters.

When will I have a bowel motion?

Usually within 1–3 days of starting. Full effect develops by day 4–7. If you have not had a motion after a week on the recommended dose, contact a healthcare professional — impaction may need a different protocol.

Can I dissolve it in juice or tea?

Cold water is the standard solvent. Cold or room-temperature flavoured liquids are acceptable but the texture may be unpleasant. Hot liquids may degrade the product over time.

Is it safe long-term?

Yes. PEG 3350 is approved for long-term use in chronic constipation. Decades of data show no electrolyte or renal harm at standard doses. The included electrolyte mix protects against the net sodium loss that would occur with PEG alone.

Can I use it in pregnancy?

Yes. PEG 3350 has no measurable absorption and is considered safe throughout pregnancy and breastfeeding. It is the preferred laxative in this population.

Will it cause electrolyte problems?

At standard doses (up to 3 sachets/day) no. The sachet electrolyte mix is calibrated to be isotonic with the colon. Very-high-dose protocols (impaction, bowel prep) need fluid intake and, in some patients (renal impairment, heart failure), electrolyte monitoring.

Why dissolve it in 125 mL water exactly?

That volume balances the electrolyte concentration so the resulting solution is isotonic with the colon lumen. Diluting it more or less changes the osmotic gradient.

Can I take it with my thyroid medication?

Take the levothyroxine first thing on an empty stomach with plain water. Wait at least 1 hour before drinking the PEG sachet. PEG accelerates transit and can reduce thyroid absorption if taken simultaneously.

Can children take it?

Yes — for children 2–11 years there is a paediatric strength (Movicol Paediatric or equivalent). Do not give the adult sachet to children < 12 years without medical advice.

What if I have stomach cramps?

Common in the first few days. Splitting the dose (one sachet morning, one evening) and gradual titration usually resolves it. If cramps persist, reduce to one sachet/day.

Below 25°C, dry place. Keep sachets sealed until use. Do not store reconstituted solution > 6 hours at room temperature.
